FEE PER SESSION:

All therapy sessions are $175 for 45-60 minutes. All diagnostic assessments and psychological testing are $190 per hour. This includes all related services including administration, scoring, documentation review, report writing, and results discussion.

EDUCATIONAL GROUP FEES:

SUCCESS is $80 per 2 hour session for 26 weekly sessions.

Command and Control is $75 per 1.5 hour session for 13 weekly sessions.

Command and Control groups fees are to be paid in 2 installments. The first installment for 7 sessions plus a a 1 time material fee of $45 will be $570. The second installment for 6 sessions will be $450.

CBID is $65 per session for 9 weeks plus a one time material fee of $65. There are also 5 driving simulator practice sessions for $40 per session (45 minutes each).

INSURANCE PLANS:

Many insurance plans are accepted.  If the IMI clinician is “IN-NETWORK”, then services will be billed directly to the insurance company and the treatment charge will be the copayment amount per your specific insurance policy.

If the IMI clinician is OUT-OF-NETWORK for PPO or Health Savings Accounts, then full payment is required until the insurance deductible is met.  After the deductible is met, co-insurance agreements apply.  IMI will bill PPO/HSA insurance plans directly when able; otherwise, required documentation (i.e. Superbill receipt) will be provided for submission and reimbursement.

FEE IS DUE & PAYABLE:

All fees are due at the time of each session.  Payment/Co-payment is accepted in cash, personal checks, or money orders (online payment options coming soon).  Payment for evaluation services will be rendered at time of services and then any remainder amount is due prior to the release of a final report.  There will be a $20 surcharge for each returned check.

CANCELLATION: 

24-hour notice is required for all cancelled appointments.  The full $175 fee will be assigned in the event of a no show without prior cancellation.  Insurance plans will not cover the charges for no show appointments.
